Basically, when you die in No Man’s Sky, you’ll lose all of the items you were carrying, unless you go to retreive them from your death location. If you die on a planet you’ll lose only the items that were in your Exosuit Inventory. Everything in your Starship will remain in the ship.

EDIT: Don't try to teleport to the second galaxy if you joined a game in the first, you will appear in the middle of the space and will die by cosmic rays, losing all your equipment.
